Retirement forms and Death Benefit forms are prepared specifically for a Participant or Beneficiary. For this reason, the Retirement forms and Death Benefit forms are not available through this website.

    General

  • Address Change Packet – It is very important that the Administrative Office has the Participant's/Beneficiary's updated address for Plan correspondence. If the Participant/Beneficiary recently moved, please complete all necessary components of this Address Change Packet and return it to the Administrative Office.
  • Authorization to Release Information - Pension Plan – Complete this form if you would like to authorize a person or entity to receive Pension information on your behalf.
  • EFT Authorization – Electronic Funds Transfer Authorization is ONLY for Participants/Beneficiaries in pay status. If the Participant would like to electronically deposit his/her monthly benefit from the Producer-Writers Guild of America Pension Plan to the Participant's bank account, please complete this form and return it to the Administrative Office. Please contact our office here for this form.
  • Designation of Beneficiary Form – This Form is designed for Participants, not receiving a pension, who want to designate their beneficiary. Please complete this Form and return it to the Administrative Office if the Participant wants to update or designate his/her beneficiary.
  • Annual Funding Notice - Plan Year 2023 – This notice includes important information about the funding status of the Producer-Writers Guild of America Pension Plan (“the Plan”) and general information about the benefit payments guaranteed by the Pension Benefit Guaranty Corporation (“PBGC”)m a federal insurance agency. All traditional pension plans (called “defined benefit pension plans”) must provide this notice every year regardless of their funding status. This notice does not mean that the Plan is terminating. It is provided for information purposes and you are not required to respond in any way. This notice is for the plan year beginning January 1, 2023 and ending December 31, 2023 (“2023 Plan Year”).
  • Annual Funding Notice - Plan Year 2022 – This notice includes important information about the funding status of the Producer-Writers Guild of America Pension Plan (“the Plan”) and general information about the benefit payments guaranteed by the Pension Benefit Guaranty Corporation (“PBGC”)m a federal insurance agency. It is provided for information purposes and you are not required to respond in any way. This notice is for the plan year beginning January 1, 2014 and ending December 31, 2014 (“2014 Plan Year”).
  • Tax Forms

    Participants receiving benefits from the Pension Plan should use these forms to change their tax withholding. Participants should type in or print the information on the form, print out the form(s), sign, and return to the Administrative Office. Tax forms can be mailed, faxed, or e-mailed to [email protected].
  • EDD DE-4P – This form is designed for a California resident who is receiving Pension or Annuity Payments. Please fill out this form and return it to the Administrative Office if the Participant is a California resident and wants to elect State tax withholding from his/her monthly benefit. Participants may also use this form if they do not want any withholding from their payments
  • IRS W-8BEN – This form is designed for a non-U.S. citizen or other non-U.S. person, including a non-resident alien who is receiving Pension or Annuity payments. Please fill out this form and return it to the Administrative Office.
  • IRS W-4P – This form is designed for U.S. Citizens, resident aliens (or their estates) who are receiving Pension or Annuity payments. If the Participant wishes to withhold Federal taxes from the monthly benefit, please fill out this form and return it to the Administrative Office. Participants may also use this form if they do not want any withholding from their payments.
  • Divorce

  • Model Qualified Domestic Relations Order (Participant Not In Pay Status) – This is a model court order that can be filed in a divorce action so that an ex-spouse can receive a portion of the Participant's benefit. This order applies if the Participant is not receiving a monthly pension benefit from the Producer-Writers Guild of America Pension Plan. Representation by an attorney is advisable.
  • Model Qualified Domestic Relations Order (Participant In Pay Status) – This is a model court order that can be filed in a divorce action so that an ex-spouse can receive a portion of the Participant's benefit. This order applies if the Participant is currently receiving a monthly pension benefit from the Producer-Writers Guild of America Pension Plan. Representation by an attorney is advisable.
  • QDRO Procedures – Please refer to the QDRO Procedures, which outlines what action the Plan will take upon receipt of a draft QDRO and other items.
