Life Event - Special Enrollment:

Q: What is a life event?
A: Life events are certain occurrences that will allow you to enroll your dependent(s) without waiting until the Fund's next Open Enrollment:

If you declined enrollment for your dependent(s) (including your spouse or same-sex domestic partnerAn individual who has submitted to the Fund an Affidavit of Domestic Partnership on a form provided by the Fund, along with supporting documentation, and who meets the criteria set forth in such Affidavit. Generally, for a partner to qualify, both the participant and his/her same-sex partner must acknowledge being in a committed relationship which has been in existence for at least six months. For more information, contact the Administrative Office.) because of other insurance coverage, you may enroll your dependent(s) in the Health Fund, without waiting until the Funds next Open Enrollment, provided that you request enrollment (including submission of a completed Dependent Enrollment Form) within 30-days after the "life event" occurs (in this example the life event is the coverage ending).

New Dependent(s):
If you have a new dependent as a result of marriage, birth, adoption, placement for adoption, or guardianship, you may enroll your dependent(s) in the Health Fund, without waiting until the next Open Enrollment, provided that you request enrollment (including submission of a completed Dependent Enrollment Form and all required documentation within 30 days after the "life event" occurs (in this example the life event is the marriage, birth, adoption, placement for adoption or guardianship).
