I hereby authorize Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund to charge the specified one-time amount to the specified credit or debit card and the financial institution for the credit or debit card, specified above, to charge my account and remit payment.
All refunds will be credited to the same card. Any failure by Producer-Writers’ Guild of America Pension Plan/Writers’ Guild Industry Health Fund to process payment does not relieve me from the responsibility of payment.
In the event Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund is unable to secure funds from my credit/debit card for this transaction for any reason, including but not limited to, inability to validate the card, invalidity of the card, insufficient funds in my credit/debit card account or insufficient or inaccurate information provided by me when I submit my electronic payment, I understand that my account may be suspended and/or Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund may generate invoices for payment and/or further collection action may be undertaken by Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund including application of fees to the extent permitted by law. A late payment may result, in the sole discretion of Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund, in accrued interest and/or late fees
These Terms and Conditions may be updated or amended by Producer-Writers’ Guild of America Pension Plan/Writers’ Guild-Industry Health Fund at any time.
